Dear Families:


Welcome back to school!  We hope you had a relaxing and enjoyable summer.  We realize that the first few days of school can be overwhelming.  It will take all of us some time to adjust, and get back into the “swing of things.” 


Most of the information contained in this packet is available on our website and can be printed and filled out if you have misplaced any items.  Our website is: http://www.andrewave.naugatuck.k12.ct.us/  

In addition, we will be sending school notices home primarily via e-mail.  Some forms may require a parent signature and will have to be printed out by you and returned completed.  Our policies and guidelines are posted on the website. Some of the items included in the packet today include 


STUDENT PROFILE FORM:  We will be collecting student & family contact information in  electronic form this year, please go to this link.  https://naugatuckschools.formstack.com/forms/student_contacts, This form is what the school relies on in the event of an emergency and must be on file for each student.  Remember that if any of these phone numbers change at any time, you must notify the office immediately.   Please keep the office updated with any changes as they occur. The link can also be found on our Andrew Avenue webpage http://www.andrewave.naugatuck.k12.ct.us/   If you do not have access to a computer it can be done here at Andrew Ave and/or  at any school in the district.

HEALTH QUESTIONNAIRE/ EMERGENCY CARE:  Please fill out and return to allow our nurse to keep her records up to date. 


SMILEBUILDERS:  an application for this program is included.  If you are interested, please complete and return.


LUNCH PROGRAM:  Information about our breakfast/lunch program is attached.  


INTERNET USAGE AGREEMENT:  Each student must have this form completed and returned before 

they are permitted to use the school’s network.
